When to use it

Use this tool to help you structure a fast and effective change initiative proposal in your organization.

What’s the purpose

Change is crucial but it can be difficult. This template guides you through the process of managing change effectively and efficiently by considering why it’s happening, who is impacted, what the change is intended to achieve and how the process will be managed to ensure success.

How to do it

  1. Start by giving your change journey a name and identifying who it’s for.
  2. Next, following the numbered order, fill in the template blocks using the prompts below to guide you:Core change team: Identify all the individuals that you need to get buy-in from to support the change process.

    Urgency: What are the top reasons why the change needs to take place? Consider the current capability to successfully execute on the change.

    Change recipients: Who is impacted by the change, directly or indirectly? Consider how you might incorporate them into the parts of the change management process that they are affected by most.

    Vision: What is the ideal future state that you are trying to achieve through the change? Create a high-level concept pitch that the team is aligned on and identify the behaviors that will support the change.

    Communication channels: What existing and new communication channels will be used to communicate the change to recipients?

    Success criteria: How will you evaluate whether a change has been accepted or rejected? How will you measure the success of an implemented change?

    Consolidation: Identify which actions are working and what you anticipate the benefits of the change to be. Consider both qualitative and quantitative benefits.

    Investments: What barriers to success do you foresee? Consider budget, time, capacity and resources.

    Actions: Following the SMART principle, identify the key actions that need to be taken and by whom in order to execute the change process successfully.
